Aging, sun  damage, weight fluctuations,  bone density loss, and other  genetic  and  life style  factors  contribute to facial aging.  These factors cause volume loss  as well as  superficial and deep  wrinkles. The aesthetic features of the upper part of the face are as follows. The forehead should be slightly convex in the women. It is desired that the area between the eyebrows, which is called the glabella, is  smooth.  The temples  should be flat or slightly  convex. In women,  if the  eyebrows  are higher than the bones at the upper edge of the orbital bones, it results in a more aesthetic appearance. The middle part of the eyebrows should be lower than the outer sides. The outer sides of the eyebrows should rise slightly upwards. In men, the eyebrows are lower than the women. If the outer sides of the eyebrows are in line with the inner sides of the eyebrows or slightly higher, it provides a better view in men.  If the vertical and  horizontal  lines between the eyebrows are very deep and do not  disappear  completely  with  botox treatment, filler can be applied to  this area.  Similarly, if horizontal  forehead wrinkles do not disappear despite the application of botox, filling treatment can be performed. With the effect of gravity and aging, the soft tissues on the temples decrease  and sag, and the temples get a collapsed appearance. These problems can be treated with fillers.

Aesthetic Filler Applications in the Middle Part of the Face

The middle part of the face contains special places such as raised cheekbones and plump cheeks, which are important for facial beauty and a youthful look. There is a  slight  convexity from  the lower eyelids towards the cheeks and lips on a young middle face,  and  the  junction of the cheeks and lower eyelid is flat.

Fillers  can  be  applied  on cheeks to increase volume and increase  projection,  especially  on  cheekbones.  Also, the teartrough at the junction of the lower eyelids and cheeks can be treated with fillers.  There may be depressions or evenwrinkles at the junction of the ear and cheek, and this area can be treated with filling application.

The nose is the most important point of the middle face. With aging,  the  appearance  of  the  nose  changes: the tip of the nose sags;  the  hump  on  the  dorsum  of  the  nose  becomes  evident; the relative  shortening  on  the  lower  face  is  combined  with  therelative  sagging on the tip of  the  nose ,  and  the  nose  looks  very elongated.  Aesthetic filler applications on the nose   is  also called  medical  rhinoplasty.  Nasal fillers, especially, have started to be applied successfully recently. However, the  size of the nose should be kept in mind. If  it is performed with an excessive amount of filler on normal sized or large noses, this will definitely enlarge the nose and give it a more repulsive  appearance. Filler  applications  should  be  carried out  in  small  amounts  in  large  noses. In particular, it is  useful for  correcting  aesthetic  problems  in  small  noses or  hiding the remaining  deformities after rhinoplasty.

Aesthetic Filler Applications in the Lower Part of the Face

The  lips are inspected against certain aesthetic criteria. The junction line of the red skin of the lips  (mucosa) and the normal skin is  examined.  A white  and  slightly raised skin line (white skin roll)  should  exist  at this border.  It should continue  from the middle of  the  upper  lip toward the columella in the form of  two lines, expressed as the philtral   columns.

The height of the lips is checked from  front view. Finally,  side view projection of the lips is noted. The upper lip is considered as three parts, the lower lip as two parts.

Evaluation of the lower part of the face should include the harmony of the  teeth  in  the  upper   and lower jaw, the lateral fullness of the jaw, and the protrusion  of the jaw.  There  must be an angle between the chin and the neck (ideally 120 degrees),  and it should  go  straight without the interruption of sagging.

The lines, also called Marionette lines, which continue from the lip edges toward the tip of the chin and make the person look sad and unhappy, are noted.

Wrinkles on the neck are checked. The condition of the bands of the platysma muscle on the neck is examined when the patient speaking and not speaking.

If the transition between red lip skin and normal skin is uncertain, this line is raised  with  filler.  If the lip height is low at the front view, its height can be increased  by filler.  In addition,  Gummy Smile problem can be treated increasing the upper lip height with filler.  When the side projection of thelips is not enough, the projection can be increased by filler. Since the lip is a very mobile structure with thin skin and since it comes into contact  with  hot and cold substances, fillers applied on lips may be less permanent  than on  other  parts of the face.

Enlargement of the angulus mandible gives a more masculine appearance, so this area can be augmented with filler in people who need it. With the filler, the chin can be enlarged, extended;  the transition between the chin tip and the middlepart of the chin can be filled,  and a straight line can be obtained.

Aesthetic Filler Applications to Improve the Skin Quality of the Face

Such fillers are applied to the skin of face, neck, hand, and over the  breastbone. These fillers also contain hyaluronic acid. They are generally preferred to moisturize and thicken the skin and increase its elasticity. They also reduce the visibility of  fine  wrinkles.  Its effect  can  last up to 8-12 months.
